Optimum Plan Management [OPM] will collect information from you that is necessary for:
- The management of your NDIS plan and budget.
- That enables us to monitor our quality service and customer satisfaction.
- For invoicing the NDIA for the services, we provide
- As required by law
- For management of our services.

Optimum Plan Management [OPM] collects personal information from you in a variety of ways, including:
- When you interact with us electronically or in person
- When we provide our services to you.
- Directly from your provider, or third party once you give authorisation.

Third parties where you consent to the use or disclosure

To the NDIA and the NDIS Quality and Safeguards Commission if needed.
To service providers to provide appropriate services. This may include third party service providers such as IT service providers, data storages, webhosting, and server providers. This may include parties located, or store data, outside of Australia.
Third parties to collect and process data, such as Google Analytics or other relevant businesses.
As part of a corporate transaction such as a sale, divestiture, merger or acquisition and/or for any other business – related purposes.
To any of our employees, insurers, advisers, suppliers, and subcontractors insofar as reasonably necessary for the purpose set out in this policy.
By providing personal information to us, you consent to us collecting, holding, and disclosing your information in accordance with this Privacy Policy. Where we disclose your personal information to third parties, we will request that the third party follow this policy regarding handling, and disclosure of your personal information.
Storage, Security, and confidentiality of your information:

Staff are bound by confidentiality. When your Personal Information is no longer needed for the purpose, it was obtained, we will take reasonable steps to destroy or permanently de identify your Personal Information. However, most of the Personal Information will be stored and kept for a minimum of 7 years or as relevant Australian legislation changesmay require.
